Affiliates form don't work

Aloha, I tried to use the affiliates (AFAD) page but it doesn't work. Neither the messages doesn't. I tried to find the pages IAFL and AFAE that are in the form but I haven't been able to find it.

Below is the code for the form.

In order to use the affiliate program it first needs to be turned on. Under Marketing, then settings there is a checkbox to activate the affiliate program. This will allow the form to be submitted.
